Appointments & Cancellation:
If you need to cancel or reschedule your appointment, please do so at least 24 hours in advance to avoid being charged a missed appointment fee. Call our office at: 602-266-6888.

Health Insurance
Biltmore Eye Physicians currently accepts most healthcare plans through United Healthcare and the BlueCross BlueShield. While we do not accept other insurance plans, we provide patients with the paperwork needed to submit reimbursement for care.

Payment for services is the responsibility of the patient, not the insurance company. Please verify insurance coverage before your appointment. When you arrive, you’ll be asked to provide your insurance card for our records.
